For pharmaceutical manufacturers, wastewater management has always been a critical operational consideration given its close connection to environmental compliance, process control, site resilience and responsible manufacturing. However, the recast EU Urban Wastewater Treatment Directive (UWWTD) represents a significant shift in how wastewater challenges are addressed and who is expected to contribute to managing them.[1]
The Directive introduces an increased focus on micropollutants - substances present in very small concentrations that can still have environmental impacts - and introduces an Extended Producer Responsibility (EPR) scheme requiring that certain producers, including companies placing medicinal products on the market, must contribute towards the cost of removing these pollutants from urban wastewater.
Understandably, there has been discussion across the pharmaceutical sector about implementation, cost allocation and potential impacts on competitiveness and access to medicines. As these discussions continue, one point is already becoming increasingly clear: the long-term direction is towards greater environmental responsibility and improved removal of micropollutants from wastewater.

A New Chapter in Wastewater Responsibility
The recast Urban Wastewater Treatment Directive entered into force on 1 January 2025 with the aim of strengthening wastewater management across Europe by improving treatment standards and addressing emerging pollutants that traditional wastewater treatment processes may not remove effectively. [2] Historically, wastewater treatment has primarily focused on removing organic matter, nutrients and other conventional pollutants. And while these processes remain essential, scientific understanding has evolved and attention has shifted more toward micropollutants.
Micropollutants are substances found in wastewater at very low concentrations that may still affect aquatic ecosystems or human health depending on their characteristics. They can include pharmaceutical residues, personal care product ingredients, industrial chemicals, microplastics and substances.
For pharmaceutical manufacturers, pharmaceutical residues are particularly relevant. Medicines play a vital role in society, and some environmental presence of active pharmaceutical ingredients is unavoidable due to normal human use, while other substances may arise from manufacturing activities and require careful management. The pharmaceutical industry has already invested significantly in wastewater treatment, process controls and environmental risk management and many manufacturing sites operate sophisticated systems to ensure process wastewater is responsibly managed before discharge.
The recast Directive, however, addresses wastewater treatment at a broader municipal level. It introduces requirements for additional treatment stages, known as quaternary treatment, to remove a wider range of micropollutants from urban wastewater. Quaternary treatment is an additional wastewater treatment step designed to target contaminants that conventional biological treatment may not remove effectively. Depending on the wastewater characteristics, this may involve technologies such as advanced biological processes, activated carbon, ozonation or other advanced treatment approaches, thus representing a significant evolution in wastewater management.[3]
Understanding Extended Producer Responsibility and the Polluter Pays Principle
One of the most significant changes introduced by the Directive is the creation of an Extended Producer Responsibility scheme. Indeed, EPR is not a new concept; it has already been applied in areas such as packaging, batteries and electronic equipment. The underlying principle is that producers should take responsibility for the environmental impacts associated with their products, including contributing towards measures required to manage those impacts.
Under the recast UWWTD, certain pharmaceutical and cosmetic producers placing products on the EU market will be required to contribute financially towards the costs associated with removing micropollutants from wastewater. These costs include investment in treatment infrastructure, operation and maintenance of treatment systems, monitoring requirements and other activities connected with the EPR scheme.
This approach is based on the “polluter pays” principle. Put simply, those contributing to an environmental impact should help fund the measures required to prevent, reduce or manage that impact rather than placing the full burden on taxpayers.
The application of this principle has generated lively discussion within the pharmaceutical sector, with industry groups raising questions around fairness, proportionality and the scientific basis for allocating costs - particularly given that micropollutants can originate from multiple sources.[4]
Such discussions are important because a sustainable approach should recognise the complexity of wastewater pollution and ensure responsibility is allocated appropriately. At the same time, the underlying objective of the Directive remains clear: reducing micropollutants in wastewater and improving environmental protection.
And so, for pharmaceutical manufacturers, the key question is not only what future obligations may look like, but how companies can prepare strategically for a future where environmental performance is increasingly connected to operational success.
The Direction Is Clear
Recent political discussions around aspects of the Directive touch upon concerns raised by some Member States and industry groups regarding cost allocation, competitiveness and potential impacts on medicine supply. It’s important to acknowledge these concerns as good regulation requires careful consideration of evidence, impacts and implementation challenges. That said, companies should not interpret these discussions as a reason to delay action.
The Directive has not been cancelled or abandoned. The broader policy direction remains focused on reducing micropollutants, strengthening wastewater treatment and increasing producer responsibility. Ireland must transpose the Directive into national law by 31 July 2027, with EPR obligations expected to become operational from the end of 2028.[5] And while this may appear some way off, wastewater strategies and infrastructure decisions require careful planning. Understanding wastewater characteristics and selecting appropriate treatment approaches cannot be done effectively last minute. In order to prepare adequately, pharmaceutical manufacturers should begin with mapping and understanding their current position:
- What is in the wastewater?
- Which compounds are present?
- How effective are existing treatment processes?
- Where are opportunities to reduce environmental impact at source?
The responses to the questions will serve as a crucial starting point in identifying the right pathway for future action. After all, if you fail to prepare, you’re preparing to fail!
